Zhewitra 40 mg (vardenafil) should be used with great caution or avoided in people with significant liver disease because the liver is responsible for metabolizing the drug. Impaired liver function can increase vardenafil levels in the body, raising the risk of side effects such as low blood pressure, dizziness, fainting, and prolonged effects. General guidance: Mild liver impairment: Lower starting doses are often recommended, and a 40 mg dose may be too high. Moderate liver impairment (for example, many patients with Child-Pugh Class B cirrhosis): Vardenafil should be used cautiously, Zhewitra 40 at reduced doses under medical supervision. Severe liver impairment (such as Child-Pugh Class C cirrhosis or advanced liver failure): Vardenafil is generally not recommended or should be avoided because safety data are limited and drug accumulation can occur. Additional reasons to avoid or seek specialist advice before using vardenafil include: Active liver failure Recent hospitalization for liver-related complications Significant jaundice Severe fluid retention (ascites) Hepatic encephalopathy Concurrent medications that strongly affect liver metabolism A 40 mg dose is particularly concerning because it exceeds the commonly recommended maximum approved daily dose of vardenafil in many jurisdictions (typically 20 mg once daily). If a product labeled "Zhewitra 40 mg" is being considered, it's important to verify the formulation and discuss it with a healthcare professional, especially if liver disease is present.
